Urgent Referrals
SEVERE CHRONIC SINUSITIS WITH UNCONTROLLED ASTHMA/ ALLERGIES
NASAL TUMOUR / MASS/ MUCOCELE - NOT MUCOUS RETENTION CYST
ODONTOGENIC SINUSITIS
SEVERE ACUTE RHINOSINUSITIS WITH EXPECTED COMPLICATION
CHRONIC SINUSITIS NOT AMENABLE TO MEDICAL THERAPY (IE. TWICE DAILY NASAL RINSES WITH NASAL STEROIDS FOR MINIMUM 6-8 WEEKS)

WE KINDLY ASK THAT YOU SEE YOUR PRIMARY CARE PHYSICIAN FOR ANY “URGENT” CONCERNS THAT ARE NOT ASSOCIATED WITH RECENT SURGERY
ALL APPOINTMENTS MUST BE CONFIRMED (AUTOMATIC CONFIRMATION SYSTEM IS USED FOR ACCURACY)
ALL MISSED APPOINTMENTS WILL BE CHARGED $150 OR WILL BE DISCHARGED FROM THE PRACTICE
IF YOU ARE ON SURGICAL WAITING LIST FOR SINUS SURGERY, YOU WILL BE NOTIFIED ONCE DATE AVAILABLE

IF YOU HAVE BEEN REFERRED TO OUR OFFICE, DO NOT CALL TO REQUEST INFORMATION ON APPOINTMENT TIMING, ETC.

Priority is given to referrals from other otolaryngologists, pulmonary physicians and allergists. Referrals from Family physicians WILL ONLY BE accepted if CT scan evidence of chronic rhinosinusitis following adequate medical therapy (CT scan should not be ordered until adequate trial of medical therapy)
